Note: There are two live scan forms. 

Commission on Teacher Credentialing Live Scan Form: This form is for first time credential applicants only. If you hold a California Credential or a Certificate of Clearance do not use this form. The fingerprint clearance will be transmitted to the California Commission on Teacher Credentialing (CCTC) only. 

Tri-County Substitute Teacher Consortium (TCSTC) Live Scan Form: This form should be used by first time substitutes who hold a California Credential or a Certificate of Clearance or are in the process of obtaining the afore mentioned, who want to substitute within the Tri-County area.  The fingerprint clearance will be transmitted to TCSTC only.
